How to Request Additional VA Community Care Services

If a veteran needs more visits, a different service, or care beyond the current authorization dates, submit a Request for Services (RFS) to the VA for review. Start early enough to allow review before the authorization ends. Processing times vary, so do not rely on a fixed turnaround estimate.

Complete the requested veteran, provider, current authorization, and service details. Attach clinical documentation that supports the request, such as a progress summary, treatment plan, relevant notes, objective measures when appropriate, and the rationale for additional care. Follow the instructions on the current form and those of the VA facility handling the referral. Submitting an RFS does not extend an authorization. Do not provide additional services outside the approved dates or scope until a new or amended authorization is received. If the existing authorization already expired, contact Help Therapy and the VA about the next step; a late request is not guaranteed to be accepted or approved.
